Participant Waiver - Release of Liability Waiver

By enrolling for programs offered by Jack of Sports Inc and/or Jack of Sports Foundation on behalf of yourself and your family members, you read, understand and agree to the following:

1. Although every effort is made to send campers/students/participants home with all of their belongings, Jack of Sports Inc or Jack of Sports Foundation is not responsible for any loss or damage.

2. Jack of Sports Inc or Jack of Sports Foundation reserves the right to cancel a child/student’s participation in the program session if their behavior is deemed unmanageable or dangerous to themselves or others, in which case they will be sent home immediately.

3. You recognize that your participation in the program/activity for which you have registered may include a risk to your health or a risk of injury. You hereby willingly assume such health risk or risk of injury for yourself and the named person(s) for whom you are in law responsible, and you assume full responsibility before, during and after your/their participation in the program/activity. You, for yourself, your heirs, executors, administrators, successors and assigns HEREBY RELEASE, WAIVE, AND FOREVER DISCHARGE the Jack of Sports Inc and Jack of Sports Foundation, all other organizations, associations and companies associated with any of the programs offered by Jack of Sports Inc or Jack of Sports Foundation, and all their respective employees, agents, contractors, consultants, representatives, and successors (all of whom are called the "Jack of Sports Inc and/or Jack of Sports Foundation’s Indemnities") OF AND FROM ALL claims, demands, losses, damages, costs, actions and other proceedings whatsoever, whether in law, statute or equity, in respect of death, injury, loss or damage to you or your property, howsoever caused, except to the extent caused by or attributable to the negligent or intentional acts of Jack of Sports Inc and/or Jack of Sports Foundation Indemnities, arising or to arise by reason of your participation in the program or any of its associated activities.

4. In the event your child is injured, ill or in need of medical attention and you are unable to be contacted, you authorize Jack of Sports Inc or Jack of Sports Foundation staff to seek medical attention on your behalf.
